href=\"\/Pain_Behaviours\" title=\"Pain Behaviours\">douleur<\/a> ou d&rsquo;un dysfonctionnement au niveau d&rsquo;un <a href=\"\/Tendon_Anatomy\" title=\"Tendon Anatomy\">tendon<\/a>, en pr\u00e9sence ou en l&rsquo;absence d&rsquo;une pathologie structurelle. <sup id=\"cite_ref-:0_1-0\" class=\"reference\"><a href=\"#cite_note-:0-1\">(1)<\/a><\/sup> <sup id=\"cite_ref-:4_2-0\" id=\"cite_ref-:4_2-1\" class=\"reference\"><a href=\"#cite_note-:4-2\">(2)<\/a><\/sup><\/p>\n<p>** Le terme tendinopathie fait r\u00e9f\u00e9rence \u00e0 la d\u00e9g\u00e9n\u00e9rescence ou \u00e0 l&rsquo;\u00e9chec de cicatrisation d\u00fb \u00e0 une surcharge continue sans p\u00e9riode de r\u00e9cup\u00e9ration ad\u00e9quate. <sup id=\"cite_ref-4\" class=\"reference\"><a Ces changements peuvent se produire non seulement au niveau structurel, mais aussi au niveau cortical. <sup id=\"cite_ref-:3_5-0\" class=\"reference\"><a href=\"#cite_note-:3-5\">(5)<\/a><\/sup> La recherche sur les tendons et la tendinopathie s&rsquo;est principalement concentr\u00e9e sur les tendons eux-m\u00eames et non sur ce qui se passe au niveau de la <a href=\"\/Spinal_cord_anatomy\" title=\"Spinal cord anatomy\">moelle \u00e9pini\u00e8re<\/a> et du <a href=\"\/Brain_Anatomy\" title=\"Brain Anatomy\">cerveau<\/a>. De m\u00eame, la force <a href=\"\/Muscle\" title=\"Muscle\">musculaire<\/a> en relation avec la tendinopathie a fait l&rsquo;objet de plus de recherches que les changements au niveau du <a href=\"\/Motor_Control_and_Learning\" title=\"Motor Control and Learning\">contr\u00f4le moteur<\/a>. Il existe plusieurs hypoth\u00e8ses et mod\u00e8les qui tentent de d\u00e9crire la pathogen\u00e8se de la tendinopathie, mais les chercheurs et les cliniciens ne savent toujours pas exactement ce qui se passe.<sup id=\"cite_ref-:1_6-0\" class=\"reference\"><a href=\"#cite_note-:1-6\">(6)<\/a><\/sup> Les tendons sont con\u00e7us pour s&rsquo;adapter aux changements de mise en charge. La <a href=\"\/Tendon_Load_and_Capacity\" title=\"Tendon Load and Capacity\">mise encharge et capacit\u00e9 des tendons<\/a> (load and capacity) est un concept selon lequel la capacit\u00e9 d&rsquo;un tendon ne peut jamais que d\u00e9passer la charge qui lui est impos\u00e9e. Lorsqu&rsquo;une charge excessive est impos\u00e9e \u00e0 un tendon, celui-ci peut devenir dysfonctionnel.<\/p>\n<p>** Plusieurs \u00e9tudes ont montr\u00e9 que les tendons ont une plus grande r\u00e9sistance \u00e0 la traction que les muscles, bien qu&rsquo;ils soient moins flexibles <sup id=\"cite_ref-7\" class=\"reference\"><a href=\"#cite_note-7\">(7)<\/a><\/sup><\/p>\n<p>En 2009, Cook et Purdam ont d\u00e9crit le mod\u00e8le du continuum tendineux, qui permet d&rsquo;\u00e9valuer la tendinopathie en fonction des modifications de la structure du tendon. <sup id=\"cite_ref-8\" class=\"reference\"><a href=\"#cite_note-8\">(8)<\/a><\/sup><\/p>\n<ul>\n<li>Le mod\u00e8le du continuum comporte trois stades : la tendinopathie r\u00e9active, le tendon remani\u00e9 (montrant des anomalies de r\u00e9paration) et la tendinopathie d\u00e9g\u00e9n\u00e9rative. <\/li>\n<li>Un tendon peut se trouver \u00e0 plusieurs stades en m\u00eame temps et, en fonction de l&rsquo;intervention, le tendon peut \u00e9voluer vers le haut ou vers le bas du continuum.<sup id=\"cite_ref-:1_6-1\" class=\"reference\"><a href=\"#cite_note-:1-6\">(6)<\/a><\/sup><\/li>\n<\/ul>\n<p>Les tendons pathologiques sont souvent plus \u00e9pais que leurs homologues non affect\u00e9s. Cette augmentation du diam\u00e8tre en ant\u00e9ropost\u00e9rieur est peut-\u00eatre due au fait que le tendon essaie de maintenir sa capacit\u00e9 de mise encharge. <sup id=\"cite_ref-:2_9-0\" class=\"reference\"><a href=\"#cite_note-:2-9\">(9)<\/a><\/sup> Docking et Cook, 2016, ont men\u00e9 une \u00e9tude sur la rotule et les tendons d&rsquo;Achille. Ils ont constat\u00e9 que les tendons pathologiques, tout en pr\u00e9sentant des zones de d\u00e9sorganisation, avaient une quantit\u00e9 accrue de structure fibrillaire align\u00e9e (la partie \u00ab\u00a0saine\u00a0\u00bb d&rsquo;un tendon) par rapport aux tendons non pathologiques.<sup id=\"cite_ref-:2_9-1\" class=\"reference\"><a href=\"#cite_note-:2-9\">(9)<\/a><\/sup> Cela pourrait potentiellement montrer que les tendons pr\u00e9sentant une pathologie augmentent la quantit\u00e9 de tendon sain pour tenter de maintenir leur hom\u00e9ostasie et leur capacit\u00e9 de charge.<\/p>\n<blockquote><p>\u00ab\u00a0Traitez le beigne et non le trou\u00a0\u00bb <sup id=\"cite_ref-:1_6-2\" class=\"reference\"><a href=\"#cite_note-:1-6\">(6)<\/a><\/sup><\/p><\/blockquote>\n<p>\u00c9tant donn\u00e9 que les tendons pathologiques comportent encore des parties de tendons sains, le principe de r\u00e9adaptation des tendons \u00ab\u00a0traiter le beigne et non le trou\u00a0\u00bb a \u00e9t\u00e9 sugg\u00e9r\u00e9. Tous les mouvements du corps sont repr\u00e9sent\u00e9s dans le cortex moteur primaire du cerveau. Les mouvements sont un r\u00e9sultat de l&rsquo;\u00e9quilibre entre les stimuli excitateurs et inhibiteurs. Chez les personnes souffrant de tendinopathie de la rotule, Rio et al 2015, ont montr\u00e9 que l&rsquo;excitabilit\u00e9 corticospinale \u00e9tait \u00e9lev\u00e9e.<sup id=\"cite_ref-10\" class=\"reference\"><a href=\"#cite_note-10\">(10)<\/a><\/sup> Parall\u00e8lement, il a \u00e9t\u00e9 d\u00e9montr\u00e9 qu&rsquo;il existe aussi une inhibition corticale lors de tendinopathies.<sup id=\"cite_ref-:3_5-1\" class=\"reference\"><a href=\"#cite_note-:3-5\">(5)<\/a><\/sup> En termes simples, ce ph\u00e9nom\u00e8ne est comparable \u00e0 celui d&rsquo;un apprenti conducteur qui aurait le pied sur l&rsquo;acc\u00e9l\u00e9rateur et le frein et qui appuierait sur ceux-ci en m\u00eame temps. Le fait qu&rsquo;une unit\u00e9 motrice pr\u00e9sente \u00e0 la fois un exc\u00e8s d&rsquo;excitation et un exc\u00e8s d&rsquo;inhibition constitue une strat\u00e9gie de mouvement peu utile, qui peut alt\u00e9rer le contr\u00f4le moteur de l&rsquo;ensemble de la cha\u00eene cin\u00e9tique.<\/p>\n<p>Ces changements neuroplastiques dans le cadre d&rsquo;une tendinopathie sont un concept \u00e9mergent.
