discipline qu&rsquo;est de la science de la mise en \u0153uvre, les travaux dans ce domaine ont r\u00e9cemment reconnu une distinction utile entre la durabilit\u00e9 et le maintien : <sup id=\"cite_ref-:0_1-2\" class=\"reference\"><a href=\"#cite_note-:0-1\">(1)<\/a><\/sup><\/p>\n<blockquote>\n<ul>\n<li>La <b>durabilit\u00e9<\/b>, qui fait g\u00e9n\u00e9ralement r\u00e9f\u00e9rence \u00e0 la mesure dans laquelle une intervention fond\u00e9e sur des donn\u00e9es probantes peut apporter les avantages escompt\u00e9s sur une p\u00e9riode prolong\u00e9e apr\u00e8s la fin du soutien externe de l&rsquo;organisme donateur.<\/li>\n<li>Le <b>maintien<\/b>, fait r\u00e9f\u00e9rence \u00e0 la mise en \u0153uvre continue de processus, de pratiques ou de routines de travail qui sont transmis et appris gr\u00e2ce \u00e0 une intervention.<\/li>\n<\/ul>\n<\/blockquote>\n<p>Le terme \u00ab\u00a0maintien\u00a0\u00bb sera utilis\u00e9 ici car il correspond \u00e0 une approche pratique complexit\u00e9 des interventions, la m\u00e9connaissance des programmes, les ressources humaines limit\u00e9es, le manque de soutien sur le lieu de travail pour les nouvelles interventions et la limit\u00e9e du syst\u00e8me de sant\u00e9, la mauvaise application des interventions fond\u00e9es sur des donn\u00e9es probantes, l&rsquo;implication inad\u00e9quate des responsables locaux de la mise en \u0153uvre et la rotation \u00e9lev\u00e9e du personnel ont \u00e9galement \u00e9t\u00e9 signal\u00e9s comme des obstacles \u00e0 l&rsquo;utilisation des interventions de sant\u00e9 publique.<sup id=\"cite_ref-5\" class=\"reference\"><a href=\"#cite_note-5\">(5)<\/a><\/sup><\/p>\n<p>Il existe toujours un foss\u00e9 de connaissances entre les donn\u00e9es probantes et la pratique r\u00e9elle \u00e0 tous les niveaux d&rsquo;un syst\u00e8me d&rsquo;organisation. Selon Mallidou et al, \u00ab\u00a0cette lacune peut \u00eatre co\u00fbteuse de trois fa\u00e7ons : adoption pr\u00e9matur\u00e9e ou surutilisation de traitements, prestation de soins sous-optimaux ou inutiles, et lancement de nouvelles \u00e9tudes qui peuvent ne pas \u00eatre inform\u00e9es par les derniers r\u00e9sultats de la recherche ou qui peuvent ne pas r\u00e9pondre pleinement aux besoins des utilisateurs des connaissances.\u00a0\u00bb Cette \u00e9tude a r\u00e9v\u00e9l\u00e9 que la recherche de strat\u00e9gies efficaces pour permettre un engagement significatif des parties prenantes est essentielle pour produire des changements durables dans la pratique et am\u00e9liorer les soins aux patients.<sup id=\"cite_ref-6\" class=\"reference\"><a href=\"#cite_note-6\">(6)<\/a><\/sup><\/p>\n<p>La r\u00e9flexion sur le maintien doit commencer au stade de la planification pr\u00e9alable \u00e0 la mise en \u0153uvre, en identifiant notamment les : d\u00e9terminants ou pr\u00e9dicteurs de maintien li\u00e9s aux facteurs et strat\u00e9gies organisationnels et contextuels <sup id=\"cite_ref-7\" class=\"reference\"><a href=\"#cite_note-7\">(7)<\/a><\/sup> et les r\u00e9sultats du maintien de l&rsquo;utilisation de l&rsquo;intervention. <sup id=\"cite_ref-8\" class=\"reference\"><a href=\"#cite_note-8\">(8)<\/a><\/sup> Pour en savoir plus sur l&rsquo;\u00e9tape de pr\u00e9-mise en \u0153uvre, veuillez consulter <a href=\"\/Implementation_Science:_Pre-Implementation_Stage\" title=\"Implementation Science: Pre-Implementation Stage\">cet article<\/a>.<\/p>\n<p>Au cours de la phase de maintien, l&rsquo;intervention est ancr\u00e9e dans l&rsquo;organisation, y compris par un financement stable et des processus permanents de suivi et\/ou d&rsquo;assurance qualit\u00e9.
