respiratoire\" href=\"\/Respiratory_System\">syst\u00e8me respiratoire<\/a> comprend les voies respiratoires, les <a title=\"Lung Anatomy\" href=\"\/Lung_Anatomy\">poumons<\/a> ainsi que les structures musculosquelettiques de la cage <a title=\"Thoracic Anatomy\" href=\"\/Thoracic_Anatomy\">thoracique<\/a>. Une mani\u00e8re simple de comprendre le syst\u00e8me respiratoire est de le diviser en deux parties : la \u00ab\u00a0pompe musculosquelettique\u00a0\u00bb et l&rsquo; \u00ab organe d&rsquo;\u00e9change gazeux \u00bb.<sup id=\"cite_ref-:0_1-0\" class=\"reference\"><a href=\"#cite_note-:0-1\">(1)<\/a><\/sup> La pompe musculosquelettique comprend les structures osseuses environnantes (<a title=\"Ribs\" href=\"\/Ribs\">cage thoracique<\/a>, rachis cervical et thoracique, r\u00e9gion pelvienne sup\u00e9rieure) ainsi que les <a title=\"Muscles of Respiration\" href=\"\/Muscles_of_Respiration\">muscles respiratoires<\/a>. L&rsquo;organe d&rsquo;\u00e9change gazeux d\u00e9signe les voies a\u00e9riennes qui permettent le mouvement de l&rsquo;air de l&rsquo;ext\u00e9rieur du corps vers les <a title=\"Alveoli\" href=\"\/Alveoli\">alv\u00e9oles<\/a>, et la membrane <a title=\"Alveoli\" href=\"\/Alveoli\"> alv\u00e9olo<\/a>capillaire, qui est l&rsquo;interface sp\u00e9cialis\u00e9e entre l&rsquo;air inspir\u00e9 et la circulation.<sup id=\"cite_ref-:0_1-1\" class=\"reference\"><a href=\"#cite_note-:0-1\">(1)<\/a><\/sup> La respiration implique l&rsquo;\u00e9change de gaz entre l&rsquo;atmosph\u00e8re et les poumons, les poumons et le <a title=\"Blood Physiology\" href=\"\/Blood_Physiology\">sang<\/a>, et le sang et les tissus corporels. La fonction de ce syst\u00e8me est de maintenir l&rsquo;apport d&rsquo;oxyg\u00e8ne dans l&rsquo;organisme et l&rsquo;\u00e9limination du dioxyde de carbone de celui-ci. <sup id=\"cite_ref-:0_1-2\" class=\"reference\"><a href=\"#cite_note-:0-1\">(1)<\/a><\/sup> La d\u00e9faillance de l&rsquo;un des composants du syst\u00e8me respiratoire peut entra\u00eener la n\u00e9cessit\u00e9 d&rsquo;une <a title=\"Ventilation and Weaning\" href=\"\/Ventilation_and_Weaning\">ventilation<\/a> m\u00e9canique.<sup id=\"cite_ref-:1_2-0\" class=\"reference\"><a href=\"#cite_note-:1-2\">(2)<\/a><\/sup><\/p>\n<p>Le <a title=\"Respiratory Failure\" href=\"\/Respiratory_Failure\">dysfonctionnement respiratoire<\/a> est un probl\u00e8me courant chez les personnes gravement malades dans les <a title=\"The Intensive Care Unit\" href=\"\/The_Intensive_Care_Unit\">unit\u00e9s de soins intensifs<\/a> (USI).<sup id=\"cite_ref-:1_2-1\" class=\"reference\"><a href=\"#cite_note-:1-2\">(2) <\/a><\/sup><sup id=\"cite_ref-:2_3-0\" class=\"reference\"><a href=\"#cite_note-:2-3\">(3)<\/a><\/sup> Les patients gravement malades en USI sont souvent confin\u00e9s \u00e0 leur lit, ce qui entra\u00eene un d\u00e9conditionnement g\u00e9n\u00e9ral ayant un impact sur de nombreux syst\u00e8mes de l&rsquo;organisme, y compris le syst\u00e8me respiratoire.<sup id=\"cite_ref-:0_1-3\" class=\"reference\"><a href=\"#cite_note-:0-1\">(1)<\/a><\/sup> Les complications pulmonaires, premi\u00e8re cause de morbidit\u00e9 hospitali\u00e8re, sont donc une pr\u00e9occupation majeure avec les patients en USI.<sup id=\"cite_ref-:0_1-4\" class=\"reference\"><a href=\"#cite_note-:0-1\">(1)<\/a><\/sup> Les complications courantes apr\u00e8s l&rsquo;alitement comprennent la r\u00e9duction des <a title=\"Lung Volumes\" href=\"\/Lung_Volumes\">volumes pulmonaires<\/a> et du d\u00e9bit expiratoire, la diminution de la force des muscles respiratoires, la r\u00e9duction de la <a title=\"Lung Compliance\" href=\"\/Lung_Compliance\">compliance<\/a> des poumons et de la paroi thoracique, ainsi que l&rsquo;alt\u00e9ration des \u00e9changes gazeux.<sup id=\"cite_ref-:0_1-5\" class=\"reference\"><a href=\"#cite_note-:0-1\">(1)<\/a><\/sup> Tous ces facteurs entra\u00eenent une augmentation du travail respiratoire du patient et du risque pour celui-ci de d\u00e9velopper une pneumonie et de l&rsquo;<a title=\"Atelectasie\" href=\"\/Atelectasis\">at\u00e9lectasie<\/a>. La faiblesse des muscles respiratoires, associ\u00e9e \u00e0 la r\u00e9duction du <a title=\"Lung Volumes\" href=\"\/Lung_Volumes\">volume courant<\/a>, peut affecter le m\u00e9canisme de la toux, ce qui augmente plus encore le risque d&rsquo;<a class=\"mw-redirect\" title=\"Infectious Disease\" href=\"\/Infectious_Disease\">infection<\/a> pulmonaire.<sup id=\"cite_ref-:0_1-6\" class=\"reference\"><a href=\"#cite_note-:0-1\">(1)<\/a><\/sup> Les patients en USI sont toujours \u00e0 risque de d\u00e9velopper des complications telles que la r\u00e9tention de s\u00e9cr\u00e9tions, l&rsquo;<a title=\"Atelectasie\" href=\"\/Atelectasis\">at\u00e9lectasie<\/a>, la <a title=\"Pneumonie\" href=\"\/Pneumonia\">pneumonie <\/a> acquise sous ventilation m\u00e9canique, la diminution de l&rsquo;expansion pulmonaire, l&rsquo;augmentation du travail respiratoire, l&rsquo;inad\u00e9quation ventilation-perfusion, la faiblesse des muscles inspiratoires et l&rsquo;\u00e9chec du sevrage de la ventilation m\u00e9canique.<sup id=\"cite_ref-:0_1-7\" class=\"reference\"><a href=\"#cite_note-:0-1\">(1) <\/a><\/sup><sup id=\"cite_ref-:2_3-1\" dysfonctionnement respiratoire<\/a> en USI sont de maintenir ou augmenter le volume pulmonaire, d&rsquo;am\u00e9liorer l&rsquo;oxyg\u00e9nation et la ventilation, d&rsquo;\u00e9liminer les s\u00e9cr\u00e9tions pour d\u00e9gager les voies respiratoires, de r\u00e9duire le travail respiratoire, de maintenir ou am\u00e9liorer la force des muscles inspiratoires afin de pr\u00e9venir les complications respiratoires chez les patients intub\u00e9s et ceux qui respirent spontan\u00e9ment.<sup id=\"cite_ref-:1_2-2\" class=\"reference\"><a href=\"#cite_note-:1-2\">(2)<\/a><\/sup><sup long terme et l&rsquo;am\u00e9lioration <a title=\"Quality of Life\" href=\"\/Quality_of_Life\">de la qualit\u00e9 de vie \u00e0 long terme<\/a> et la r\u00e9duction de la dur\u00e9e du s\u00e9jour
