B. L\u00e4ufer, sowie sedent\u00e4re (bewegungsarme) Menschen mittleren bis h\u00f6heren Alters (40-60 Jahre) mit hohem Body-Mass-Index.<sup id=\"cite_ref-:0_1-5\" class=\"reference\"><a href=\"#cite_note-:0-1\">(1)<\/a><\/sup><sup id=\"cite_ref-:6_9-1\" class=\"reference\"><a href=\"#cite_note-:6-9\">(9)<\/a><\/sup> Der plantare Fersenschmerz kann langwierig sein und die Lebensqualit\u00e4t erheblich beeintr\u00e4chtigen. Au\u00dferdem ist er mit signifikanten Behinderungen verbunden, da die Schmerzen allt\u00e4gliche und sportliche Aktivit\u00e4ten erschweren.<sup id=\"cite_ref-:3_4-2\" class=\"reference\"><a href=\"#cite_note-:3-4\">(4)<\/a><\/sup><sup id=\"cite_ref-:6_9-2\" class=\"reference\"><a href=\"#cite_note-:6-9\">(9)<\/a><\/sup><\/p>\n<p>Trotz der deutlichen pers\u00f6nlichen Beeintr\u00e4chtigungen und der gesellschaftlichen Belastung, die mit dieser Erkrankung verbunden sind, ist die \u00c4tiologie des plantaren Fersenschmerzes nach wie vor unklar.<sup id=\"cite_ref-:1_2-1\" class=\"reference\"><a href=\"#cite_note-:1-2\">(2)<\/a><\/sup><sup id=\"cite_ref-:5_8-2\" class=\"reference\"><a href=\"#cite_note-:5-8\">(8)<\/a><\/sup> Die Diagnose wird mit einer Reihe von Faktoren auf der Ebene der F\u00fc\u00dfe in Verbindung gebracht, wie z. B.:<\/p>\n<ul>\n<li>Fu\u00dftyp mit Tendenz zur Pronation<\/li>\n<li>eingeschr\u00e4nkte Dorsalextension des Sprunggelenks<\/li>\n<li>Dorsalextension des ersten MTP-Gelenks (Gro\u00dfzehengrundgelenk)<\/li>\n<li>verminderte Muskelkraft im Fu\u00df und im Sprunggelenk<\/li>\n<\/ul>\n<p>Sowie Faktoren auf pers\u00f6nlicher Ebene wie:<\/p>\n<ul>\n<li>hoher Body-Mass-Index<\/li>\n<li>Depression;<\/li>\n<li>\u00c4ngste;<\/li>\n<li>Stress;<\/li>\n<li>Berufe, die langes Stehen erfordern<sup id=\"cite_ref-:1_2-2\" class=\"reference\"><a href=\"#cite_note-:1-2\">(2)<\/a><\/sup><sup id=\"cite_ref-:5_8-3\" class=\"reference\"><a href=\"#cite_note-:5-8\">(8)<\/a><\/sup><\/li>\n<\/ul>\n<p>Diese Faktoren lassen zwar keinen R\u00fcckschluss auf die Ursache zu, unterst\u00fctzen aber die Ansicht, dass der plantare Fersenschmerz ein komplexes und multifaktorielles Problem ist.<sup id=\"cite_ref-:1_2-3\" class=\"reference\"><a href=\"#cite_note-:1-2\">(2)<\/a><\/sup><sup id=\"cite_ref-:5_8-4\" class=\"reference\"><a href=\"#cite_note-:5-8\">(8)<\/a><\/sup> Die Komplexit\u00e4t des plantaren Fersenschmerzes in Verbindung mit dem begrenzten Verst\u00e4ndnis der \u00c4tiologie f\u00fchrt zu Unsicherheit \u00fcber die wirksamsten Interventionsstrategien.<sup id=\"cite_ref-:1_2-4\" class=\"reference\"><a href=\"#cite_note-:1-2\">(2)<\/a><\/sup><\/p>\n<h2><span id=\"Location_of_the_Pain\" class=\"mw-headline\">Lokalisation der Schmerzen <\/span><span class=\"mw-editsection\"><span class=\"mw-editsection-bracket\">(<\/span> <a class=\"mw-editsection-visualeditor\" title=\"Edit section: Location of the Pain\" href=\"\/index.php?title=Introduction_to_Plantar_Heel_Pain&amp;veaction=edit&amp;section=4\">edit<\/a> <span class=\"mw-editsection-divider\"> | <\/span> <a title=\"Edit section: Location of the Pain\" href=\"\/index.php?title=Introduction_to_Plantar_Heel_Pain&amp;action=edit&amp;section=4\">edit source<\/a> <span class=\"mw-editsection-bracket\">)<\/span><\/span><\/h2>\n<p>Klinisch gesehen sind Schmerzen die h\u00e4ufigsten Beschwerden beim plantaren Fersenschmerz. Sie f\u00fchren zu Schwierigkeiten bei gewichtstragenden T\u00e4tigkeiten wie Gehen und Stehen und wirken sich negativ auf berufliche T\u00e4tigkeiten und Freizeitaktivit\u00e4ten aus.<sup id=\"cite_ref-:7_10-0\" class=\"reference\"><a href=\"#cite_note-:7-10\">(10)<\/a><\/sup> Der Schmerz ist typischerweise an der anteromedialen Seite der plantaren Ferse lokalisiert.<sup id=\"cite_ref-:0_1-6\" class=\"reference\"><a href=\"#cite_note-:0-1\">(1)<\/a><\/sup> Kliniker verwenden bei der manuellen Palpation der Ferse h\u00e4ufig den Daumendruck, um den genauen Ort des Schmerzes zu lokalisieren und eine Diagnose zu stellen.<sup id=\"cite_ref-:7_10-1\" class=\"reference\"><a href=\"#cite_note-:7-10\">(10)<\/a><\/sup> Der Druck auf die F\u00fc\u00dfe wird seit langem als ein wichtiger Faktor in der \u00c4tiologie des plantaren Fersenschmerzes angesehen.<sup id=\"cite_ref-:1_2-5\" class=\"reference\"><a href=\"#cite_note-:1-2\">(2)<\/a><\/sup><\/p>\n<p>Saban und Masharawi <sup id=\"cite_ref-:7_10-2\" class=\"reference\"><a href=\"#cite_note-:7-10\">(10)<\/a><\/sup> schlugen die Verwendung von Druckschmerzschwellen (Pressure Pain Thresholds &#8211; PPTs) als halb-objektive Methode zur Quantifizierung des Ausma\u00dfes des mechanischen Drucks vor, der erforderlich ist, um Schmerzen auszul\u00f6sen.<\/p>\n<ul>\n<li>Die Druckschmerzschwelle wird definiert als &#8222;der geringste Druck, bei dem das Druckempfinden in Schmerz umschl\u00e4gt&#8220; <sup id=\"cite_ref-:2_3-2\" class=\"reference\"><a href=\"#cite_note-:2-3\">(3)<\/a><\/sup> und kann leicht mit der Druckalgometrie getestet werden.<sup id=\"cite_ref-:7_10-3\" class=\"reference\"><a href=\"#cite_note-:7-10\">(10)<\/a><\/sup><\/li>\n<li>Die Druckschmerzschwellen sind im Allgemeinen in den normal belasteten Teilen des Fu\u00dfes h\u00f6her, was eine geringere mechanische Empfindlichkeit ausdr\u00fcckt. Die Ferse weist demnach die h\u00f6chsten Werte der plantaren Seite des Fu\u00dfes auf, was mit dem Belastungsmuster der Ferse beim Gehen \u00fcbereinstimmt.<sup id=\"cite_ref-:2_3-3\" class=\"reference\"><a href=\"#cite_note-:2-3\">(3)<\/a><\/sup><sup id=\"cite_ref-:7_10-4\" class=\"reference\"><a href=\"#cite_note-:7-10\">(10)<\/a><\/sup><\/li>\n<li>Um die Druckschmerzschwelle zu messen, positionierten Saban und Masharawi <sup id=\"cite_ref-:7_10-5\" class=\"reference\"><a href=\"#cite_note-:7-10\">(10)<\/a><\/sup> den Patienten in R\u00fcckenlage mit den F\u00fc\u00dfen \u00fcber der Bettkante und ohne Druck auf die Ferse. Der Fersenbereich wurde in 5 Regionen unterteilt und die PPT-Messungen wurden an jeder dieser Stellen mit einem computergest\u00fctzten mechanischen Handdruckalgometer aufgezeichnet.<\/li>\n<li>In ihrer Studie stellten Saban und Masharawi<sup id=\"cite_ref-:7_10-6\" class=\"reference\"><a href=\"#cite_note-:7-10\">(10)<\/a><\/sup> fest, dass der anteromediale Aspekt der Ferse der empfindlichste Bereich der Ferse ist, was mit dem von den Patienten h\u00e4ufig genannten Bereich \u00fcbereinstimmt.
