Auf Diabetes entfallen 9 % der gesamten Gesundheitskosten f\u00fcr Erwachsene.<sup id=\"cite_ref-:2_2-0\" class=\"reference\"><a href=\"#cite_note-:2-2\">(2)<\/a><\/sup><\/p>\n<p>Die periphere Neuropathie ist eine h\u00e4ufige Folge von Diabetes,<sup id=\"cite_ref-3\" class=\"reference\"><a href=\"#cite_note-3\">(3)<\/a><\/sup> mit einer Pr\u00e4valenz von 10 bis 85 %.<sup id=\"cite_ref-:2_2-1\" class=\"reference\"><a href=\"#cite_note-:2-2\">(2)<\/a><\/sup> Die Folgen der peripheren Neuropathie k\u00f6nnen verheerend sein und umfassen (1) Ulzera am Fu\u00df, (2) schwere Amputationen, (3) St\u00fcrze, (4) intrakranielle Verletzungen und (5) eine verminderte Lebensqualit\u00e4t.<sup id=\"cite_ref-:7_1-1\" class=\"reference\"><a href=\"#cite_note-:7-1\">(1)<\/a><\/sup> Ungef\u00e4hr einer von vier Menschen mit Diabetes entwickelt ein chronisches diabetisches Fu\u00dfulkus. Menschen mit einem diabetischen Fu\u00dfulkus haben ein 2,5-mal h\u00f6heres Sterberisiko als Menschen mit Diabetes, die kein Fu\u00dfulkus entwickeln; Sterblichkeitsrate steigt innerhalb von f\u00fcnf Jahren nach Auftreten eines diabetischen Fu\u00dfulkus auf 42 %. Allgemein ausgedr\u00fcckt bestehen die drei Funktionen des ZNS darin, (1) sensorische und sensible Informationen aufzunehmen, (2) diese Informationen zu verarbeiten und (3) motorische Signale auszusenden. \u00dcber diese Mechanismen steuert das ZNS die meisten K\u00f6rperfunktionen, einschlie\u00dflich der Bewegung, der Wahrnehmung durch unsere f\u00fcnf Sinne und h\u00f6herer Funktionen wie Kognition, Bewusstsein und Sprache. Das R\u00fcckenmark ist eine Verl\u00e4ngerung des Gehirns und dient als Nervenbahn f\u00fcr den Informationsaustausch mit dem Rest des K\u00f6rpers.<\/p>\n<p><b>Peripheres Nervensystem<\/b>: ein komplexes Netz von Nerven, die afferente Informationen (sensorisch und sensibel) aus der Peripherie des K\u00f6rpers \u00fcber das R\u00fcckenmark <i>in<\/i> das ZNS leiten und efferente Informationen <i>aus<\/i> dem ZNS \u00fcber das R\u00fcckenmark an die Peripherie weitergeben. Beispiele f\u00fcr efferente Signale, die \u00fcber das periphere Nervensystem (PNS) \u00fcbertragen werden, sind (1) motorische Informationen f\u00fcr die Muskelaktivit\u00e4t und (2) autonome Funktionen (Herzfrequenz, Blutdruck, Atmung, Verdauung, sexuelle Erregung).<sup id=\"cite_ref-:0_4-0\" class=\"reference\"><a href=\"#cite_note-:0-4\">(4)<\/a><\/sup><\/p>\n<ul>\n<li><b>Motorische Nerven<\/b>: leiten Informationen an die Skelettmuskeln und das somatische Gewebe weiter, wodurch willk\u00fcrliche Bewegungen entstehen<\/li>\n<li><b>Sensorische und sensible Nerven<\/b>: leiten Informationen aus der Umwelt \u00fcber die Sinnesorgane und Rezeptoren im K\u00f6rper an das ZNS weiter<\/li>\n<li><b>Vegetatives (autonomes) Nervensystem:<\/b> leitet motorische Informationen an die viszeralen Organe weiter, um die glatte Muskulatur, den Herzmuskel und die Dr\u00fcsen zu innervieren, und dient der Aufrechterhaltung der Hom\u00f6ostase des K\u00f6rpers. Das vegetative Nervensystem besteht aus zwei Teilen: dem <b>Sympathikus<\/b> und dem <b>Parasympathikus<\/b>, die die viszeralen Organe innervieren. Diese Sch\u00e4digung \u00e4u\u00dfert sich zun\u00e4chst in einer Fehlfunktion der Nerven aufgrund von zellul\u00e4ren und chemischen Ver\u00e4nderungen. Mit der Zeit wird diese neurale Fehlfunktion jedoch zu einer echten Nerven- oder Struktursch\u00e4digung, die zu Atrophie und Demyelinisierung f\u00fchrt.<sup id=\"cite_ref-:2_2-3\" class=\"reference\"><a href=\"#cite_note-:2-2\">(2)<\/a><\/sup> Es gibt mehr als 100 bekannte Arten von PN, jede mit einzigartigen Symptomen und Prognosen. Die PN-Symptome h\u00e4ngen von der Kategorie der betroffenen Nerven ab: motorisch, sensorisch\/sensibel oder autonom.<sup id=\"cite_ref-:0_4-1\" class=\"reference\"><a href=\"#cite_note-:0-4\">(4)<\/a><\/sup><\/p>\n<p>Die genaue Pathophysiologie der PN h\u00e4ngt von den zugrunde liegenden Krankheitsprozessen ab. Die Mechanismen der peripheren Nervensch\u00e4digung weisen jedoch \u00e4hnliche Muster auf.
