Daher sind f\u00fcr die Anpassung dieser Hilfsmittel mehr Geschick und Erfahrung erforderlich. genannt, wird zur Kontrolle der Innen- und Au\u00dfenrotation eingesetzt. Diese weniger bekannte Orthese besteht aus einem AFO-Teil, einem freien Kniegelenk und einem Beckenteil zur Beeinflussung der Rotation an der H\u00fcfte. Diese Hilfsmittel werden am h\u00e4ufigsten bei Patienten mit Neuralrohrdefekt eingesetzt und in der Regel bei Kindern im Alter von etwa 2 oder 3 Jahren angepasst.<sup id=\"cite_ref-:0_1-4\" class=\"reference\"><a href=\"#cite_note-:0-1\">(1)<\/a><\/sup> Eine tibiale Au\u00dfentorsion wird h\u00e4ufig bei Menschen mit <a title=\"Spina Bifida\" href=\"\/Spina_Bifida\">Spina bifida<\/a> beobachtet. Zu den Kompensationsbewegungen, die aus einer tibiale Au\u00dfentorsion resultieren, geh\u00f6ren:<sup id=\"cite_ref-2\" class=\"reference\"><a href=\"#cite_note-2\">(2)<\/a><\/sup><\/p>\n<ul>\n<li>Rumpfneigung zur Standphase<\/li>\n<li>Dynamische Beckenrotation<\/li>\n<li>Innenrotation der H\u00fcfte<\/li>\n<li>Flexion des Knies in der Standphase<\/li>\n<li>Valgus im Sprunggelenk und im R\u00fcckfu\u00df<\/li>\n<\/ul>\n<p>Dar\u00fcber hinaus weisen Kinder mit <a class=\"mw-redirect\" title=\"Spina Bifida Occulta\" href=\"\/Spina_Bifida_Occulta\">Spina bifida<\/a> typischerweise eine Schw\u00e4che der H\u00fcftextensoren und -abduktoren auf, was zu kompensatorischen Bewegungen an anderen Stellen f\u00fchrt. Diese Art von Orthesen wird im Allgemeinen aus folgenden Gr\u00fcnden verordnet:<sup id=\"cite_ref-:0_1-5\" class=\"reference\"><a href=\"#cite_note-:0-1\">(1)<\/a><\/sup><\/p>\n<ul>\n<li>Instabilit\u00e4t des Knies<\/li>\n<li>Muskelschw\u00e4che im Bereich Sprunggelenk \/ Knie \/ H\u00fcfte<\/li>\n<li>Kontraktur des Knies<\/li>\n<li>Hyperextension des Knies<\/li>\n<li>Kontrolle von Fu\u00df und Sprunggelenk<\/li>\n<li>Zur Unterst\u00fctzung der H\u00fcfte<\/li>\n<\/ul>\n<p>Ein wichtiger Aspekt bei der Anpassung einer KAFO ist die Beurteilung der Kraft des <a title=\"Quadriceps Muscle\" href=\"\/Quadriceps_Muscle\">M. quadriceps.<\/a> Eine Kraft &lt; 3 auf der <a title=\"Muscle Strength Testing\" href=\"\/Muscle_Strength_Testing\">Oxford-Skala<\/a> bedeutet, dass der Patient weder sein Knie in Extension noch sein K\u00f6rpergewicht w\u00e4hrend des Gehens halten kann. Daher m\u00fcsste die KAFO ein gesperrtes Kniegelenk haben.
