Betroffene leiden in der Regel unter einer Morgensteifigkeit, die l\u00e4nger als eine Stunde anh\u00e4lt, oder Steifigkeit nach Inaktivit\u00e4t; die betroffenen Gelenke k\u00f6nnen sich nach mehr als einer Stunde Inaktivit\u00e4t warm und schmerzempfindlich anf\u00fchlen. Dies stimuliert die B-Zellen, sich zu vermehren und in Plasmazellen zu differenzieren, die Autoantik\u00f6rper (d.h. CCP-Antik\u00f6rper und Rheumafaktor) produzieren. CCP-Antik\u00f6rper k\u00f6nnen nachgewiesen werden, bevor sich bei Menschen mit RA Gelenkbeschwerden entwickeln. Ihre Produktion deutet auf einen &#8222;Bruch der immunologischen Toleranz&#8220; hin.<sup id=\"cite_ref-:7_18-4\" class=\"reference\"><a href=\"#cite_note-:7-18\">(18)<\/a><\/sup><\/p>\n<p>Autoantik\u00f6rper k\u00f6nnen dann in die Gelenksynovialis wandern. Symmetrische Schwellungen in den kleinen Gelenken sind der &#8222;\u00e4u\u00dfere Ausdruck einer Entz\u00fcndung der Synovialmembran nach einer Immunaktivierung&#8220;.<sup id=\"cite_ref-:7_18-5\" class=\"reference\"><a href=\"#cite_note-:7-18\">(18)<\/a><\/sup> Sobald die Autoantik\u00f6rper in der Synovialis sind, produzieren sie proinflammatorische Zytokine (z.B. Interferon-gamma und Interleukine). Dies veranlasst Makrophagen, Monozyten und Synovialfibroblasten zur Freisetzung von Zytokinen wie Tumor-Nekrose-Faktor alpha (TNF alpha), Interleukin 6 (IL-6) und IL-1. Synovialfibroblasten beginnen dann, sich \u00fcberm\u00e4\u00dfig zu vermehren, was die Gelenkentz\u00fcndung verst\u00e4rkt (d. h. synoviale Hyperplasie). Die aktivierten Synovialzellen produzieren den Pannus, eine verdickte Synovialmembran aus Granulationsgewebe mit invasivem Wachstum. Diese Pannusbildung wird begleitet von einer Degradation von Knorpel und Knochen, wobei aktivierte Synovialzellen Proteasen produzieren, die den Knorpel abbauen. Dies produziert zus\u00e4tzliche proinflammatorische Faktoren, die bestehende Prozesse verschlimmern. Bei fortschreitender RA sind mehrere Gelenke auf beiden Seiten des K\u00f6rpers betroffen, und roinflammatorische Zytokine breiten sich auch auf andere Teile des K\u00f6rpers aus Dies verursacht extraartikul\u00e4re Symptome, wie Vaskulitis, Rheumaknoten und Proteinabbau in der Skelettmuskulatur.
