Muskeln oder der umgebenden Strukturen, w\u00e4hrend die Abk\u00fcrzung \u201eTMG\u201c das Kiefergelenk selbst (Temporomandibulargelenk) bezeichnet. <sup id=\"cite_ref-1\" class=\"reference\"><a href=\"#cite_note-1\">(1)<\/a><\/sup><sup id=\"cite_ref-:4_2-0\" class=\"reference\"><a href=\"#cite_note-:4-2\">(2)<\/a><\/sup><\/p>\n<p>Weltweit leiden zu jedem beliebigen Zeitpunkt zwischen 5 und 12 % der Menschen an TMD-Symptomen, die einer Behandlung bed\u00fcrfen (Punktpr\u00e4valenz), wobei Frauen etwa doppelt so h\u00e4ufig wie M\u00e4nner an symptomatischen TMD leiden und eine Behandlung in Anspruch nehmen.<sup id=\"cite_ref-:6_3-0\" class=\"reference\"><a href=\"#cite_note-:6-3\">(3)<\/a><\/sup> Ein weitaus gr\u00f6\u00dferer Anteil der Bev\u00f6lkerung zeigt jedoch im Laufe seines Lebens Zeichen und Symptome einer TMD (Lebenszeitpr\u00e4valenz).<sup id=\"cite_ref-:6_3-1\" class=\"reference\"><a href=\"#cite_note-:6-3\">(3)<\/a><\/sup> Es wird berichtet, dass mehr als ein Drittel der Bev\u00f6lkerung mindestens ein Zeichen oder Symptom einer TMD aufweist, wie beispielsweise Gelenkger\u00e4usche (Knacken oder Krepitation), Schmerzen bei der Palpation der Kaumuskulatur oder eine eingeschr\u00e4nkte Kieferbewegung.<sup id=\"cite_ref-:4_2-1\" class=\"reference\"><a href=\"#cite_note-:4-2\">(2)<\/a><\/sup> Diese Unterscheidung zwischen der symptomatischen Pr\u00e4valenz (Personen, die aktiv eine Behandlung suchen oder ben\u00f6tigen) und dem Vorliegen jeglicher Zeichen oder Symptome ist entscheidend f\u00fcr das Verst\u00e4ndnis des Ausma\u00dfes class=\"mw-editsection-bracket\">)<\/span><\/span><\/h3>\n<p>Der Goldstandard f\u00fcr die Diagnose von temporomandibul\u00e4ren Dysfunktionen (TMD) sind die <b>Research Diagnostic Criteria for Temporomandibular Disorders<\/b> (RDC\/TMD). Dieses Klassifikationssystem ist zwar umfassend, jedoch zeitaufwendig, weshalb es sich eher f\u00fcr den Forschungsbereich als f\u00fcr die t\u00e4gliche klinische Praxis eignet. Dennoch hilft das Verst\u00e4ndnis dieses Rahmens den Klinikern, die Komplexit\u00e4t von TMD besser einzusch\u00e4tzen.<sup id=\"cite_ref-:0_4-0\" class=\"reference\"><a href=\"#cite_note-:0-4\">(4)<\/a><\/sup><\/p>\n<p>Die RDC\/TMD unterteilen TMD in der Achse der k\u00f6rperlichen Diagnosen in zwei Hauptkategorien: schmerzbezogene und intraartikul\u00e4re St\u00f6rungen.<\/p>\n<p>Zu den <b>schmerzbezogenen St\u00f6rungen<\/b> z\u00e4hlen Myalgie und Arthralgie. Die <b>Myalgie <\/b>wird weiter unterteilt in lokale Myalgie, myofasziale Schmerzen und myofasziale Schmerzen mit \u00dcbertragung. <b>Arthralgie<\/b> bezeichnet Gelenkschmerzen, die direkt am Kiefergelenk lokalisiert sind. Die RDC\/TMD-Klassifikation ber\u00fccksichtigt zudem <b>Kopfschmerzen<\/b>, die auf TMDs zur\u00fcckzuf\u00fchren sind. Diese werden konkret definiert als Kopfschmerzen im Schl\u00e4fenbereich, die durch die Kieferfunktion verschlimmert und durch Palpation ausgel\u00f6st werden k\u00f6nnen.<sup id=\"cite_ref-:0_4-1\" class=\"reference\"><a href=\"#cite_note-:0-4\">(4)<\/a><\/sup><\/p>\n<p><b><\/b>Intraartikul\u00e4re St\u00f6rungen sind biomechanische oder strukturelle Probleme innerhalb des Gelenks. Bei einer Diskusverlagerung wird der Discus meist nach anterior disloziert.<\/p>\n<p><b>Diskusverlagerung mit Reposition<\/b>: Der dislozierte Discus kehrt beim \u00d6ffnen des Kiefers in seine normale Position zur\u00fcck, was typischerweise ein h\u00f6rbares oder tastbares Knacken verursacht.
