Cruciate Ligament (ACL)\" href=\"\/Anterior_Cruciate_Ligament_(ACL)\">vordere Kreuzband<\/a> (VKB, Ligamentum cruciatum anterius) ist eine der wichtigsten stabilisierenden Strukturen des Kniegelenks, aber auch anf\u00e4llig f\u00fcr Verletzungen. Die einzigartige Struktur des Bandes verhindert eine anteriore Translation der Tibia gegen\u00fcber dem Femur und begrenzt die tibiale Rotation und die Valgusstellung des Knies, w\u00e4hrend es gleichzeitig eine wichtige propriozeptive Funktion erf\u00fcllt. VKB-Verletzungen geh\u00f6ren zu den h\u00e4ufigsten und schwerwiegendsten Knieverletzungen im Sport, wobei etwa 70 % der F\u00e4lle auf Verletzungsmechanismen ohne direkten Kontakt zur\u00fcckzuf\u00fchren sind. Der Ansatz des Bandes verl\u00e4uft weiter unter dem Lig. transversum genus hindurch und verbindet sich mit dem Vorderhorn des Meniskus lateralis. Von der Tibia verl\u00e4uft das VKB nach posterior und lateral zu seinem proximalen Ansatz, der posterior an der medialen Seite des lateralen Femurkondylus liegt.<sup id=\"cite_ref-:0_1-0\" class=\"reference\"><a href=\"#cite_note-:0-1\">(1)<\/a><\/sup><\/p>\n<p>Die Fasern des VKB sind in einer medialen Spirale von etwa 110 Grad zwischen Tibia und Femur angeordnet. Das AMB setzt am anteromedialen Aspekt des tibialen Ansatzes an, w\u00e4hrend das PLB am posterolateralen Aspekt des tibialen Ansatzes ansetzt. W\u00e4hrend der Extension des Knies wird das PLB straff, w\u00e4hrend sich das AMB lockert. Bei der Flexion des Knies verh\u00e4lt es sich umgekehrt: Das AMB wird gestrafft und das PLB entspannt. Die Fasern des VKB sind in zwei unterschiedlichen Faszikelmustern angeordnet: Ein Typ verl\u00e4uft direkt zwischen den tibialen und femoralen Ans\u00e4tzen, w\u00e4hrend ein anderer Typ entlang der L\u00e4ngsachse des Bandes in einer Helixachse von 25 Grad spiralf\u00f6rmig verl\u00e4uft.<sup id=\"cite_ref-:0_1-3\" class=\"reference\"><a href=\"#cite_note-:0-1\">(1)<\/a><\/sup><\/p>\n<p>Diese spiralf\u00f6rmige Anordnung erm\u00f6glicht ein einzigartiges Verhalten bei der Belastung. Die erste entspricht der Belastung vor der Flie\u00dfgrenze (pre-yield loading) &#8211; die Verformung der Fasern bleibt elastisch, w\u00e4hrend sich das Band dehnt und entwindet. L\u00e4sst die Belastung nach, kehren die Fasern in ihre urspr\u00fcngliche Form zur\u00fcck. Kommt es hingegen zu einer \u00dcberlastung oberhalb der Flie\u00dfgrenze (post-yield loading), werden die Querverbindungen (Crosslinks) zwischen den Fasern aufgebrochen, was zu einer dauerhaften Verformung f\u00fchrt.<sup id=\"cite_ref-:0_1-5\" class=\"reference\"><a href=\"#cite_note-:0-1\">(1)<\/a><\/sup><\/p>\n<p>Mehrere Faktoren beeinflussen die Zugfestigkeit des VKB. Mit zunehmendem Alter nimmt die Zugfestigkeit ab. Zyklische Belastungen durch Aktivit\u00e4ten wie Gehen oder Laufen k\u00f6nnen die W\u00e4rmeproduktion erh\u00f6hen und die Flie\u00dfgrenze herabsetzen, wobei sich das Band jedoch innerhalb weniger Stunden nach einer solchen Belastung wieder erholt. Eine Ruhigstellung verringert die Zugfestigkeit mit der Zeit. Die Innenrotation beeinflusst die Zugfestigkeit, und Torsionskr\u00e4fte k\u00f6nnen m\u00f6glicherweise zu einem st\u00e4rkeren Versagen f\u00fchren als andere Belastungsmuster. Diese Rezeptoren sind an der Peripherie positioniert, wo die maximale Biegung stattfindet, und verlaufen parallel zur L\u00e4ngsachse des Bandes.
