class=\"reference\"><a href=\"#cite_note-1\">(1)<\/a><\/sup> und machen etwa 2 % der F\u00e4lle von Handverletzungen aus, die eine chirurgische Behandlung erfordern.<sup id=\"cite_ref-2\" class=\"reference\"><a href=\"#cite_note-2\">(2)<\/a><\/sup> Zu den verschiedenen Verletzungsmechanismen geh\u00f6ren Hyperflexion, direktes stumpfes Trauma und penetrierendes Trauma. Anteile. Diese sind der Mittelz\u00fcgel oder Tractus intermedius (Ansatz an der Basis der mittleren Phalanx), und zwei Seitenz\u00fcgel, die sich als terminale Sehne wieder vereinigen und an der Basis des distalen Phalanx ansetzen. Um eine aktive interphalangeale Extension zu erzeugen, ben\u00f6tigt der <a title=\"Muskel\" href=\"\/Muscle\">EDC-Muskel<\/a> die Unterst\u00fctzung von zwei intrinsischen Muskelgruppen, den Mm. interossei und den Mm. lumbricales. Diese Muskeln setzen an der <a title=\"Extensor Hood Mechanism Hand\" href=\"\/Extensor_Hood_Mechanism_Hand\">Dorsalaponeurose (&#8222;Streckerhaube&#8220;)<\/a> und den Seitenz\u00fcgeln an. Die EDC-Sehne und alle ihre komplizierten aktiven und passiven Verbindungen am und distal des Fingergrundgelenks (Metacarpophalangealgelenk, MCP-Gelenk) werden als &#8222;Streckapparat der Hand&#8220; bezeichnet. Die Grundlage des Streckapparats bilden die Sehnen des M. extensor digitorum communis (mit <a title=\"Extensor Indicis Proprius\" href=\"\/Extensor_Indicis_Proprius\">M. extensor indicis<\/a> und <a title=\"Extensor Digiti Minimi\" href=\"\/Extensor_Digiti_Minimi\">M. extensor digiti minimi<\/a>) und die Dorsalaponeurose, der Mittelz\u00fcgel und die Seitenz\u00fcgel, die in die terminale Sehne \u00fcbergehen. Die Lamina triangularis hilft, die Sehnenanteile auf dem Fingerr\u00fccken zu stabilisieren. Sind diese jedoch ebenfalls gerissen, entsteht die typische Knopflochdeformit\u00e4t am PIP-Gelenk. In diesem Fall wird die gesamte Streckkraft \u00fcber die intakten Seitenz\u00fcgel auf die distale Phalanx \u00fcbertragen, was zu einer Hyperextension des DIP-Gelenks f\u00fchrt. Das PIP-Gelenk knickt in Flexion und ragt durch die L\u00fccke in der Streckerhaube nach oben. Kontinuit\u00e4tsunterbrechung des Mittelz\u00fcgels k\u00f6nnen sich unmittelbar nach einer Verletzung des Fingers entwickeln. Unter Umst\u00e4nden k\u00f6nnen sie auch erst 10 bis 14 Tage nach der urspr\u00fcnglichen Verletzung auftreten,<sup id=\"cite_ref-:3_18-0\" class=\"reference\"><a href=\"#cite_note-:3-18\">(18)<\/a><\/sup> da es eine Weile dauern kann, bis die Streckerhaube und die Lamina triangularis \u00fcber dem PIP-Gelenk vollst\u00e4ndig rei\u00dfen.<sup id=\"cite_ref-:2_19-0\" class=\"reference\"><a href=\"#cite_note-:2-19\">(19)<\/a><\/sup> Typische Zeichen sind:<\/p>\n<ul>\n<li>Verlust der Extension am PIP-Gelenk; deutliche Hyperextension am DIP-Gelenk<sup id=\"cite_ref-:3_18-1\" class=\"reference\"><a href=\"#cite_note-:3-18\">(18)<\/a><\/sup><\/li>\n<li>ein schwacher Griff und die Unf\u00e4higkeit, kleine Gegenst\u00e4nde mit der Fingerspitze zu greifen und zu manipulieren<\/li>\n<li>Schwellung und Schmerzen dorsal \u00fcber dem PIP-Gelenk<sup id=\"cite_ref-:1_20-0\" class=\"reference\"><a href=\"#cite_note-:1-20\">(20)<\/a><\/sup><\/li>\n<\/ul>\n<h2><span id=\"Special_Tests\" class=\"mw-headline\">Spezielle Tests <\/span><span class=\"mw-editsection\"><span class=\"mw-editsection-bracket\">(<\/span><a class=\"mw-editsection-visualeditor\" title=\"Edit section: Special Tests\" href=\"\/index.php?title=Central_Slip_Extensor_Tendon_Injuries&#038;veaction=edit&#038;section=6\">edit<\/a><span class=\"mw-editsection-divider\"> | <\/span><a title=\"Edit section: Special Tests\" href=\"\/index.php?title=Central_Slip_Extensor_Tendon_Injuries&#038;action=edit&#038;section=6\">edit source<\/a><span class=\"mw-editsection-bracket\">)<\/span><\/span><\/h2>\n<p>Zu den speziellen Tests, die bei der Erkennung von Verletzungen des Streckapparats helfen, geh\u00f6ren:<\/p>\n<ol>\n<li><a title=\"Elson Test\" href=\"\/Elson_Test\">Elson-Test<\/a> \u2013 Das PIP-Gelenk wird in einem 90\u00b0-Winkel fixiert und der Patient wird aufgefordert, das DIP-Gelenk zu strecken. Ein schlaffes DIP-Gelenk trotz Streckbem\u00fchungen des Patienten bedeutet ein negatives bzw. normales Ergebnis (wenn der Mittelz\u00fcgel intakt ist, verhindern die lockeren Seitenz\u00fcgel bei Flexion des PIP-Gelenks die Extension des DIP-Gelenks). Dadurch kann die Lamina triangularis gestrafft und somit die volare Verlagerung der Seitenz\u00fcgel verhindert werden.
