(WAD) ist ein Begriff, der eine Reihe von Symptomen nach einem zervikalen bzw. kraniozervikalen Beschleunigungstrauma (nachfolgend zur Vereinfachung \u201eSchleudertrauma\u201c genannt) beschreibt. Es gilt als das h\u00e4ufigste Ergebnis nach \u201enicht katastrophalen\u201c Kfz-Unf\u00e4llen.<sup id=\"cite_ref-1\" class=\"reference\"><a href=\"#cite_note-1\">(1)<\/a><\/sup> Der Begriff WAD wird oft synonym mit dem Begriff Schleudertrauma verwendet. Das Schleudertrauma bezieht sich jedoch auf den Mechanismus der akuten Verletzung und nicht auf das Vorhandensein von Symptomen wie Schmerzen, Steifheit, Muskelkr\u00e4mpfen und Kopfschmerzen, ohne dass eine L\u00e4sion oder strukturelle Pathologie vorliegt.<sup id=\"cite_ref-2\" class=\"reference\"><a href=\"#cite_note-2\">(2)<\/a><\/sup><sup id=\"cite_ref-:19_3-0\" class=\"reference\"><a href=\"#cite_note-:19-3\">(3)<\/a><\/sup> Die Prognose des Schleudertraumas ist unbekannt und unvorhersehbar. leichte Weichteilverletzungen ohne Anzeichen einer Fraktur eingestuft.<\/p>\n<p>Die Verletzung erfolgt in drei Phasen:<\/p>\n<ul>\n<li>Phase 1: Die obere und untere HWS durchlaufen in Phase 1 eine Flexion<\/li>\n<li>Phase 2: Die HWS nimmt eine S-Form an, w\u00e4hrend sie in die Extension \u00fcbergeht und sich schlie\u00dflich aufrichtet, um die Lordose wiederherzustellen <\/li>\n<li>Stadium 3: Die gesamte HWS befindet sich in Extension, wobei eine starke Scherkraft auftritt, die zu einer Kompression der Facettengelenkkapseln f\u00fchrt <\/li>\n<\/ul>\n<p>Studien an Leichen haben gezeigt, dass das Schleudertrauma durch die S-f\u00f6rmige Kr\u00fcmmung der HWS entsteht, die zu einer Hyperextension an der unteren HWS und einer Flexion der oberen Segmente f\u00fchrt, was die physiologischen Grenzen der Wirbels\u00e4ulenbeweglichkeit \u00fcberschreitet.<\/p>\n<p>Die Quebec Task Force klassifiziert Patienten mit Schleudertrauma je nach Schweregrad der Zeichen und Symptome wie folgt:<\/p>\n<ol>\n<li>Grad 1: Der Patient klagt \u00fcber Nackenschmerzen, Steifheit oder Druckempfindlichkeit, ohne dass bei der k\u00f6rperlichen Untersuchung pathologische Befunde auffallen.<\/li>\n<li>Grad 2: Der Patient weist muskuloskelettale Befunde auf, einschlie\u00dflich eines verminderten Bewegungsausma\u00dfes und Druckempfindlichkeit.<\/li>\n<li>Grad 3: Der Patient zeigt auch neurologische Befunde, die sensible Defizite, verminderte Sehnenreflexe und Muskelschw\u00e4che umfassen k\u00f6nnen.<\/li>\n<li>Grad 4: Der Patient weist eine Fraktur auf.<sup Eine Studie von Holm et al. deutet darauf hin, dass die Zahl der Betroffenen, die \u00fcber Symptome berichten, eine steigende Tendenz aufweist. In ihrer 2008 ver\u00f6ffentlichten Studie gaben sie an, dass die Inzidenz in Nordamerika und Europa bei etwa 300 pro 100.000 Einwohner lag.<sup id=\"cite_ref-9\" class=\"reference\"><a href=\"#cite_note-9\">(9)<\/a><\/sup> Im Vereinigten K\u00f6nigreich stand die Einf\u00fchrung der Gurtpflicht im Jahr 1983, eine Initiative zur Reduzierung von Verkehrstoten, im Zusammenhang mit einem Anstieg der gemeldeten WAD-F\u00e4lle in den Folgejahren.<sup id=\"cite_ref-10\" class=\"reference\"><a href=\"#cite_note-10\">(10)<\/a><\/sup> WAD treten auch h\u00e4ufiger bei Frauen als bei M\u00e4nnern auf, wobei fast zwei Drittel der Frauen \u00fcber Symptome klagen und mehrere Studien ergaben, dass Frauen im Vergleich zu M\u00e4nnern tendenziell eine langsamere oder unvollst\u00e4ndige Genesung aufweisen.<sup id=\"cite_ref-11\" class=\"reference\"><a href=\"#cite_note-11\">(11)<\/a><\/sup><\/p>\n<p>Das Risiko, dass Patienten nach einem zervikalen Beschleunigungstrauma WAD entwickeln, h\u00e4ngt von einer Vielzahl von Faktoren ab:<\/p>\n<ul>\n<li>Schwere des Aufpralls; allerdings ist es schwierig, objektive Evidenz zu erheben, um dies zu best\u00e4tigen<sup id=\"cite_ref-:7_12-0\" class=\"reference\"><a href=\"#cite_note-:7-12\">(12)<\/a><\/sup><\/li>\n<li>Vor dem Unfall vorhandene Nackenschmerzen sind ein Risikofaktor f\u00fcr akute Nackenschmerzen nach einem Aufprall<sup id=\"cite_ref-:8_13-0\" class=\"reference\"><a href=\"#cite_note-:8-13\">(13)<\/a><\/sup> <\/li>\n<li>Frauen scheinen ein etwas h\u00f6heres Risiko zu haben, WAD zu entwickeln<\/li>\n<li>Auch das Alter spielt eine Rolle; j\u00fcngere Menschen (18-23) reichen eher Versicherungsanspr\u00fcche ein und\/oder haben ein h\u00f6heres Risiko, wegen WAD behandelt zu werden<sup id=\"cite_ref-:1_14-0\" class=\"reference\"><a href=\"#cite_note-:1-14\">(14)<\/a><\/sup><sup id=\"cite_ref-:9_15-0\" class=\"reference\"><a href=\"#cite_note-:9-15\">(15)<\/a><\/sup><\/li>\n<\/ul>\n<p>Die Zahl der Menschen weltweit, die unter chronischen Schmerzen leiden, liegt zwischen 2 und 58 %, meist jedoch zwischen 20 und 40 %<sup id=\"cite_ref-:8_13-1\" class=\"reference\"><a href=\"#cite_note-:8-13\">(13)<\/a><\/sup><\/p>\n<ul>\n<li>Wenn ein Patient 3 Monate nach dem Unfall immer noch Symptome hat, wird er wahrscheinlich mindestens zwei Jahre lang symptomatisch bleiben, m\u00f6glicherweise sogar noch viel l\u00e4nger<sup id=\"cite_ref-:9_15-1\" class=\"reference\"><a href=\"#cite_note-:9-15\">(15)<\/a><\/sup> <\/li>\n<li>50 % der Menschen mit einem Schleudertrauma erholen sich vollst\u00e4ndig,<\/li>\n<li>25 % k\u00f6nnen leichte Behinderungen aufweisen und der Rest leidet unter m\u00e4\u00dfigen bis starken Schmerzen und Behinderungen<sup id=\"cite_ref-:38_16-0\" class=\"reference\"><a href=\"#cite_note-:38-16\">(16)<\/a><\/sup><\/li>\n<\/ul>\n<p>Es gibt viele prognostische Faktoren, die die Entwicklung von WAD und die Wahrscheinlichkeit der Chronifizierung bestimmen.<\/p>\n<ul>\n<li>Es wurde festgestellt, dass eine geringe Erwartung an die Genesung, passive Bew\u00e4ltigungsstrategien und posttraumatische Stresssymptome mit chronischen Nackenschmerzen und\/oder Behinderungen nach einem Schleudertrauma verbunden sind<sup id=\"cite_ref-17\" class=\"reference\"><a href=\"#cite_note-17\">(17)<\/a><\/sup><\/li>\n<li>Selbstberichtete unspezifische Schmerzen vor dem Unfall, hohe psychische Belastung, weibliches Geschlecht und niedriger Bildungsstand sagten zuk\u00fcnftige selbstberichtete Nackenschmerzen voraus<sup id=\"cite_ref-:2_18-0\" class=\"reference\"><a href=\"#cite_note-:2-18\">(18)<\/a><\/sup><\/li>\n<li>Vorgeschichte fr\u00fcherer Nackenschmerzen, ein Ausgangswert der Nackenschmerzintensit\u00e4t von mehr als 55\/100, Nackenschmerzen bei der Erstuntersuchung, Kopfschmerzen bei der Erstuntersuchung, Katastrophisierung, ein WAD-Grad von 2 oder 3 und das Nichtanlegen des Sicherheitsgurts zum Zeitpunkt des Aufpralls sind weitere ung\u00fcnstige prognostische Faktoren<sup id=\"cite_ref-19\" class=\"reference\"><a href=\"#cite_note-19\">(19)<\/a><\/sup><sup id=\"cite_ref-:38_16-1\" class=\"reference\"><a href=\"#cite_note-:38-16\">(16)<\/a><\/sup><\/li>\n<li>Ebenso, wenn der Patient vor dem Unfall nicht gearbeitet hat, krankgeschrieben war oder Sozialhilfe bezogen hat<sup id=\"cite_ref-:2_18-1\" class=\"reference\"><a href=\"#cite_note-:2-18\">(18)<\/a><\/sup><\/li>\n<li>Fr\u00fche Behinderungen nach der Verletzung stehen in engem Zusammenhang mit chronischen Behinderungen, aber auch psychologische und verhaltensbezogene Faktoren spielen eine wichtige Rolle<sup id=\"cite_ref-20\" class=\"reference\"><a href=\"#cite_note-20\">(20)<\/a><\/sup><\/li>\n<li>K\u00e4ltehyperalgesie, vermindertes Bewegungsausma\u00df der HWS, Kopfschmerzen, posttraumatische Stresssymptome, \u00dcbererregungssymptome (PTBS) und anf\u00e4nglich hoher NDI (Neck Disability Index) stehen im Zusammenhang mit chronischen m\u00e4\u00dfigen bis schweren Behinderungen<sup id=\"cite_ref-:38_16-2\" class=\"reference\"><a href=\"#cite_note-:38-16\">(16)<\/a><\/sup><\/li>\n<\/ul>\n<h2><span id=\"Whiplash_Clinical_Prediction_Rule\" class=\"mw-headline\">Clinical Prediction Rule f\u00fcr Schleudertrauma <\/span><span class=\"mw-editsection\"><span class=\"mw-editsection-bracket\">(<\/span><a class=\"mw-editsection-visualeditor\" title=\"Edit section: Whiplash Clinical Prediction Rule\" href=\"\/index.php?title=Whiplash_Associated_Disorders&amp;veaction=edit&amp;section=6\">edit<\/a><span class=\"mw-editsection-divider\"> | <\/span><a title=\"Edit section: Whiplash Clinical Prediction Rule\" href=\"\/index.php?title=Whiplash_Associated_Disorders&amp;action=edit&amp;section=6\">edit source<\/a><span class=\"mw-editsection-bracket\">)<\/span><\/span><\/h2>\n<p>Eine Clinical Prediction Rule (CPR, klinische Vorhersageregel) ist ein Instrument, mit dem sich das Ergebnis vorhersagen l\u00e4sst, z. Physiotherapeuten neigten dazu, die Ergebnisse ihrer Patienten &#8222;\u00fcberm\u00e4\u00dfig optimistisch&#8220; einzusch\u00e4tzen. Bei einem akuten Schleudertrauma dauern die Symptome nicht l\u00e4nger als 2\u20133 Monate an, w\u00e4hrend sie bei einem chronischen Schleudertrauma l\u00e4nger als drei Monate anhalten. Bei Patienten mit akuten WAD kommt es zu einer weit verbreiteten Druckempfindlichkeit und einer verminderten Beweglichkeit der Halswirbels\u00e4ule.<sup id=\"cite_ref-:13_37-0\" class=\"reference\"><a href=\"#cite_note-:13-37\">(37)<\/a><\/sup> Verschiedene Studien deuten darauf hin, dass es innerhalb von 2\u20133 Monaten zu einer Spontanheilung kommen kann.<sup id=\"cite_ref-38\" class=\"reference\"><a href=\"#cite_note-38\">(38)<\/a><\/sup> Nach Angaben der Quebec Task Force of WAD (QTF-WAD) erholen sich 85% der Patienten innerhalb von 6 Monaten.<sup id=\"cite_ref-:4_39-0\" class=\"reference\"><a href=\"#cite_note-:4-39\">(39)<\/a><\/sup> <\/p>\n<p>Dar\u00fcber hinaus waren laut einer Folgestudie von Crutebo et al. Diese k\u00f6nnen von den Patienten subjektiv berichtet werden und umfassen Allodynie, starke Schmerzempfindlichkeit, K\u00e4lteempfindlichkeit und Schlafst\u00f6rungen aufgrund von Schmerzen.<\/p>\n<p>Objektiv k\u00f6nnen bei der neurologischen Untersuchung verminderte Reflexe, eine verminderte Muskelkraft (Myotome) und sensible Defizite (Dermatome) festgestellt werden. Diese Reaktionen k\u00f6nnen unabh\u00e4ngig von der psychischen Belastung auftreten. Dies hat im Vergleich zu Ruhe und der Verwendung einer Halskrause eine bessere Wirkung auf die <a title=\"Pain Behaviours\" href=\"\/Pain_Behaviours\">Schmerzen<\/a>, die Beweglichkeit der Halswirbels\u00e4ule und die Genesung. B. eine passive Mobilisation, die an der HWS angewendet wird, einen gewissen Nutzen bei der Schmerzlinderung haben kann.<sup id=\"cite_ref-:34_47-3\" class=\"reference\"><a href=\"#cite_note-:34-47\">(47)<\/a><\/sup> <\/li>\n<\/ul>\n<ul>\n<li>Patienten mit WAD der Grade 1 und 2 zeigten gute Ergebnisse in einem multimodalen Behandlungsprogramm, das Gruppentherapie, manuelle Therapie, Edukation und \u00dcbungen umfasste. Bei der Nachuntersuchung nach 6 Monaten gaben 65 % der Probanden eine vollst\u00e4ndige R\u00fcckkehr zur Arbeit an, 92 % gaben eine teilweise oder vollst\u00e4ndige R\u00fcckkehr zur Arbeit an und 81 % gaben an, dass sie \u00fcber 6 Monate keine medizinischen oder paramedizinischen Behandlungen erhalten hatten.<sup id=\"cite_ref-60\" class=\"reference\"><a href=\"#cite_note-60\">(60)<\/a><\/sup><\/li>\n<\/ul>\n<ul>\n<li>Die Verwendung einer Halskrause steht im Gegensatz zu den Empfehlungen der meisten Studien, die Aktivierung, Mobilisation und Bewegung empfehlen.
