Sie arbeiten phasisch, d.h. im Ein-\/Aus-Modus, und ihre Bewegung ist richtungsabh\u00e4ngig. Bei einer Dysfunktion kommt es zu Spasmus, Schmerzen und eingeschr\u00e4nktem Bewegungsausma\u00df. M. trapezius, M. levator scapulae, M. sternocleidomastoideus, die Mm. splenii und die Skalenusgruppe sind oberfl\u00e4chliche Muskeln, die im zervikalen Bereich als globale Mobilisatoren wirken.<\/p>\n<ol>\n<li>Oberfl\u00e4chliche Nackenflexoren: M. sternocleidomastoideus und M. scalenus anterior<\/li>\n<li>Oberfl\u00e4chliche Nackenextensoren:\n<ul>\n<li>1. Schicht: M. levator scapulae, M. trapezius pars descendens <\/li>\n<li>2. Schicht: M. splenius capitis und M. splenius colli<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n<p><b><\/b>Lokale Stabilisatoren sind die tiefer liegenden Muskeln, die die segmentale Kontrolle und die neutrale Position der Gelenke steuern. Ihre Aktivit\u00e4t ist eher tonisch als phasisch und sie arbeiten \u00fcber lange Zeitr\u00e4ume mit einer viel niedrigeren Kontraktionsst\u00e4rke. Dysfunktionen in den lokalen Stabilisatoren f\u00fchren zu einer Hemmung (Inhibition) der Funktion, verz\u00f6gertem Timing oder Rekrutierungsdefiziten und dem Verlust der segmentalen Kontrolle und der neutralen Gelenkpositionierung.<\/p>\n<p>Es gibt <b>drei Hauptgruppen<\/b> von zervikalen Muskeln, die eine \u201eManschette\u201c um die Wirbels\u00e4ule bilden und posturale Kontrolle und segmentale Bewegungen erm\u00f6glichen (lokale Stabilisatoren):<\/p>\n<ol>\n<li><b><\/b>Die tiefen Nackenflexoren \u2013 M. longus colli, M. longus capitis, M. rectus capitis anterior und M. rectus capitis lateralis<\/li>\n<li><b>Die tiefen Nackenextensoren<\/b> \u2013 M. semispinalis capitis, M. semispinalis colli (cervicis) und Mm. multifidi (segmentale Stabilisatoren).<\/li>\n<li><b>Die kurzen Nackenmuskeln \/ Mm. suboccipitales<\/b> \u2013 Mm. rectus capitis posterior major et minor, Mm. obliquus capitis inferior et superior<sup id=\"cite_ref-9\" class=\"reference\"><a href=\"#cite_note-9\">(9)<\/a><\/sup><\/li>\n<\/ol>\n<p>Die subokzipitale Muskelgruppe spielt eine wichtige Rolle bei der <a title=\"Propriozeption\" href=\"\/Proprioception\">Propriozeption<\/a> und liefert Informationen an das visuelle und das vestibul\u00e4re System. Sie steuert die kraniozervikale Lordose sowie kleine Kopf-Hals-Bewegungen. Eine Dysfunktion f\u00fchrt zu sensomotorischen Beeintr\u00e4chtigungen und einer Ver\u00e4nderung der Kin\u00e4sthesie, einschlie\u00dflich eines verminderten <a title=\"Gleichgewicht\" href=\"https:\/\/langs.physio-pedia.com\/de\/balance-de\/\">Gleichgewichts<\/a> und Gelenkstellungssinns (Joint Position Sense) sowie einer ver\u00e4nderten <a title=\"Oculomotor Nerve\" href=\"\/Oculomotor_Nerve\">okulomotorischen Kontrolle<\/a>. Dies kann zu zervikogenem Schwindel f\u00fchren.<\/p>\n<h2><span id=\"Cervical_Muscle_Function_in_the_Presence_of_Pain\" class=\"mw-headline\">Funktion der zervikalen Muskulatur bei Schmerzen <\/span><span class=\"mw-editsection\"><span class=\"mw-editsection-bracket\">(<\/span><a class=\"mw-editsection-visualeditor\" title=\"Edit section: Cervical Muscle Function in the Presence of Pain\" href=\"\/index.php?title=Muscle_Performance_in_Neck_Pain&#038;veaction=edit&#038;section=9\">edit<\/a><span class=\"mw-editsection-divider\"> | <\/span><a title=\"Edit section: Cervical Muscle Function in the Presence of Pain\" href=\"\/index.php?title=Muscle_Performance_in_Neck_Pain&#038;action=edit&#038;section=9\">edit source<\/a><span class=\"mw-editsection-bracket\">)<\/span><\/span><\/h2>\n<p>Eine verringerte Aktivit\u00e4t der tiefen Nackenflexoren tritt zusammen mit einer erh\u00f6hten Aktivit\u00e4t und neuromuskul\u00e4ren Ineffizienz der oberfl\u00e4chlichen Muskeln auf, selbst bei geringer Belastung und bei nicht funktionellen Aufgaben.<sup id=\"cite_ref-:0_1-1\" class=\"reference\"><a href=\"#cite_note-:0-1\">(1)<\/a><\/sup> <sup id=\"cite_ref-:6_10-0\" class=\"reference\"><a href=\"#cite_note-:6-10\">(10)<\/a><\/sup> <sup id=\"cite_ref-:1_11-0\" class=\"reference\"><a href=\"#cite_note-:1-11\">(11)<\/a><\/sup> Diese Ver\u00e4nderung der Aktivierung kann sowohl bei funktionellen als auch bei kognitiven Aufgaben beobachtet werden und ist nicht abh\u00e4ngig von der Ursache oder der Dauer der Nackenschmerzen.<sup id=\"cite_ref-:0_1-2\" class=\"reference\"><a href=\"#cite_note-:0-1\">(1)<\/a><\/sup> <sup id=\"cite_ref-:1_11-1\" class=\"reference\"><a href=\"#cite_note-:1-11\">(11)<\/a><\/sup> Patienten mit Nackenschmerzen f\u00e4llt es schwer, die oberfl\u00e4chlichen Nackenflexoren zu entspannen, selbst wenn die Aktivit\u00e4t beendet ist.<sup id=\"cite_ref-:1_11-2\" class=\"reference\"><a href=\"#cite_note-:1-11\">(11)<\/a><\/sup><\/p>\n<p>Diese Ver\u00e4nderungen der Muskelfunktion treten kurz nach Beginn der Schmerzen auf und normalisieren sich nicht automatisch wieder, sobald die Schmerzen nachlassen.<sup id=\"cite_ref-:1_11-3\" class=\"reference\"><a href=\"#cite_note-:1-11\">(11)<\/a><\/sup> Zumal Ver\u00e4nderungen in der Aktivierung der tiefen zervikalen Muskeln einen Einfluss auf die St\u00fctzung und Kontrolle der HWS haben, kann dies zu einer \u00dcberlastung einzelner Segmente f\u00fchren.<sup id=\"cite_ref-:1_11-4\" class=\"reference\"><a href=\"#cite_note-:1-11\">(11)<\/a><\/sup><sup id=\"cite_ref-:0_1-3\" class=\"reference\"><a href=\"#cite_note-:0-1\">(1)<\/a><\/sup><\/p>\n<p>Forschungen haben gezeigt, dass sowohl die Nackenflexoren als auch die Nackenextensoren bei Nackenschmerzen an Kraft und Ausdauer verlieren,<sup id=\"cite_ref-:6_10-1\" class=\"reference\"><a href=\"#cite_note-:6-10\">(10)<\/a><\/sup><sup id=\"cite_ref-12\" class=\"reference\"><a href=\"#cite_note-12\">(12)<\/a><\/sup><sup id=\"cite_ref-13\" class=\"reference\"><a href=\"#cite_note-13\">(13)<\/a><\/sup> und dass die Co-Aktivierung insbesondere bei funktionellen Aufgaben nachl\u00e4sst.<sup id=\"cite_ref-:0_1-4\" class=\"reference\"><a href=\"#cite_note-:0-1\">(1)<\/a><\/sup> Die oberfl\u00e4chlichen Nackenmuskeln erm\u00fcden auch schneller,<sup id=\"cite_ref-:1_11-5\" class=\"reference\"><a href=\"#cite_note-:1-11\">(11)<\/a><\/sup> und die Ausdauer der zervikalen Muskulatur ist sowohl bei maximaler als auch bei schwacher Kontraktion reduziert.<sup id=\"cite_ref-:0_1-5\" class=\"reference\"><a href=\"#cite_note-:0-1\">(1)<\/a><\/sup><\/p>\n<p>Die Feed-Forward-Aktivierung geht verloren, was das Timing der Aktivierung beeinflusst. Dies ist erkennbar, wenn der Patient schnelle Armbewegungen ausf\u00fchrt:<sup id=\"cite_ref-:0_1-6\" class=\"reference\"><a href=\"#cite_note-:0-1\">(1)<\/a><\/sup> Wenn ein Patient ohne Nackenschmerzen seinen Arm schnell hebt, werden die zervikalen Muskeln innerhalb von 50 ms nach der Aktivierung des <a title=\"M. deltoideus\" href=\"\/Deltoid\">M. deltoideus<\/a> aktiviert. Bei Patienten mit Nackenschmerzen liegt hingegen sowohl bei den oberfl\u00e4chlichen als auch bei den tiefen Nackenflexoren ein signifikantes Defizit vor. Das Nicken bewirkt, dass der Druck in der aufgeblasenen Manschette ansteigt.<\/p>\n<p>In der ersten Phase des Tests sollte der Druck um 2 mmHg bis auf 22 mmHg ansteigen, w\u00e4hrend die oberfl\u00e4chlichen Muskeln stets entspannt bleiben. Der Patient wird aufgefordert, diese Position f\u00fcr 10 Sekunden aufrechtzuerhalten. Personen, die keine Nackenschmerzen haben, sind in der Lage, die Kontraktion 10 Sekunden lang auf Stufe 3 (26 mmHg) oder h\u00f6her zu halten. Anschlie\u00dfend wird der Kopf knapp von der Wand wegbewegt und dort 10 Sekunden lang gehalten. Dies wird bis zu 10 Mal wiederholt.
