Es ist immer noch nicht klar, welche Behandlungen optimal sind, und die Wahl der Interventionen h\u00e4ngt von individuellen Faktoren ab, einschlie\u00dflich der Ursache der Symptome (z. B. ven\u00f6se Stauung, Bandscheibe, die auf das Neuroforamen dr\u00fcckt, Knochenwachstum oder Bandscheibendegeneration).<sup id=\"cite_ref-:6_2-0\" class=\"reference\"><a href=\"#cite_note-:6-2\">(2)<\/a><\/sup><\/p>\n<p>Zu den aktuellen Behandlungsoptionen geh\u00f6ren invasive Verfahren (z. B. Operationen und Injektionen), nicht invasive Verfahren (z. B. Edukation, manuelle Therapie, \u00dcbungen) und die medikament\u00f6se Behandlung (z. B. nicht steroidale Antirheumatika (NSARs) und orale Steroide). Oft kann eine Kombination von Interventionen erforderlich sein.<\/p>\n<p>Denken Sie daran, dass wir die Treatment-based Classification (TBC) als Leitfaden f\u00fcr die Behandlung von Personen mit Kreuzschmerzen mit oder ohne lumbale Radikulopathie verwenden k\u00f6nnen. Bei einer lumbalen Radikulopathie besteht das Risiko einer schwerwiegenden Erkrankung wie dem <a title=\"Cauda-equina-Syndrom\" href=\"\/Cauda_Equina_Syndrome\">Cauda-Equina-Syndrom<\/a>, Krebs oder Frakturen. Unter Ber\u00fccksichtigung der Pr\u00e4valenz sollte Folgendes bedacht werden:<sup id=\"cite_ref-4\" class=\"reference\"><a href=\"#cite_note-4\">(4)<\/a><\/sup><\/p>\n<ul>\n<li>Red Flags sind in bestimmten Bereichen der Medizin h\u00e4ufiger anzutreffen (z.B. wird ein Wirbels\u00e4ulenchirurg mehr Personen mit Red Flags sehen als ein Physiotherapeut)<\/li>\n<li>\u00c4rzte in der Notaufnahme werden mehr Patienten mit schweren Wirbels\u00e4ulenerkrankungen sehen als Haus\u00e4rzte<\/li>\n<\/ul>\n<p>Seien Sie sich bewusst, dass ein einzelnes Red-Flag-Symptom nicht unbedingt auf eine ernsthafte Pathologie der Wirbels\u00e4ule zur\u00fcckzuf\u00fchren ist. Au\u00dferdem deuten einige Symptome und Informationen auf die Notwendigkeit einer umfassenderen Beurteilung hin, darunter:<sup id=\"cite_ref-5\" class=\"reference\"><a href=\"#cite_note-5\">(5)<\/a><\/sup><\/p>\n<ul>\n<li>ein k\u00fcrzlich erlittenes Trauma, vor allem bei Personen \u00fcber 50 Jahren: dies erh\u00f6ht das Risiko einer Wirbelfraktur<\/li>\n<li>Krebs in der Vorgeschichte<\/li>\n<li>Infektion vor kurzer Zeit<\/li>\n<li>Blasen- oder Darmfunktionsst\u00f6rungen (einschlie\u00dflich Harnverhalt, Verlust der Kontrolle \u00fcber die Blase \/ den Darm)<\/li>\n<li>Schmerzen, die den Patienten aus dem Schlaf wecken (bitte beachten Sie, dass dies eine hohe Falsch-Positiv-Rate hat)<\/li>\n<\/ul>\n<p>Es wurde ein <a title=\"Eine Einf\u00fchrung in Red Flags bei ernsthaften Pathologien\" href=\"\/An_Introduction_to_Red_Flags_in_Serious_Pathology\">Rahmenkonzept<\/a> entwickelt, das Kliniker bei der Suche nach Red Flags unterst\u00fctzt.<sup id=\"cite_ref-:4_6-0\" class=\"reference\"><a href=\"#cite_note-:4-6\">(6)<\/a><\/sup> Dieser Rahmen ermutigt die Kliniker, sich nicht nur auf eine einzelne Red Flag zu verlassen, sondern neben der\/den Red Flags auch den Kontext des Patienten zu bewerten und dabei Faktoren wie den Verlauf der Symptome und gleichzeitig bestehende Erkrankungen zu ber\u00fccksichtigen. Es gibt Evidenz daf\u00fcr, dass die spinale Mobilisation bei lumbaler Radikulopathie hilfreich sein kann.
